FILLING OUT AN APPLICATION - you MUST include a personal reference, and a Vet reference (if this is your first dog, please find a vet before adopting, and just note that you haven't been to this vet yet). If you rent, you must include a phone number for your landlord (absolutely NO exceptions on this one!). All applications without this information will NOT be considered. The application is to be filled out by an adult. If you have an appt to meet a dog, and have changed your mind, please notify us by email or if you were given a phone number for a foster home, call them. We have several people wanting the same dogs, and this isn't fair to them. -----------------------------------
You MUST be over 18 to fill out an application, and ALL people that will be residing with the dog MUST know you are applying for a dog/puppy.

Are you willing to be patient and will you keep the dog/puppy should accidents happen or favorite shoes and toys get chewed?
